Prevalence of Rectal Chlamydia and Gonorrhea Infection Among Men who have Sex with Men (MSM)Vancouver, Canada, 20082009

Background: Prevalence of rectal chlamydia (CT) and gonorrhea (GC) among MSM is unknown in Vancouver and screening is recommended for MSM having unprotected anal sex. Self-collected rectal swabs (SCRS), particularly outside of clinical settings, may promote testing. We incorporated SCRS into the ManCount study, a venue-based HIV surveillance survey of MSM. This study aimed to determine feasibility of self-collection, prevalence of CT and GC by HIV status and age, and history of GC testing. Methods: MSM aged ? 18 years were recruited at community venues in Vancouver from September 2008 to February 2009 and asked to provide a SCRS for CT and GC testing (using the ThinPrep specimen collection system and Gen-Probe APTIMA Combo 2 Assay). Participants had completed a dried blood spot HIV test and a self-administered questionnaire on sexual behaviours and health care. Results: SCRS were obtained from 252 of 766 survey participants; 239 (94.8%) were tested. Overall prevalence of CT was 5.4%, but varied by HIV status and age (no significant differences): HIV-negative (6.2%), HIV-positive (3.3%), age < 25 years (8.7%), age ? 25 (4.7%). Overall GC prevalence was 2.1%. This did not differ by HIV status, but was higher in MSM < 25 years (6.5% vs 1.0% in others, p=0.05). Most participants (76.9%) reported previous GC testing (86.0% of HIV-positive and 73.9% of HIV-negative MSM, p=0.08). Conclusions: One-third of participants provided a SCRS, with CT and GC prevalence comparable to other studies in MSM. Incorporating SCRS into existing venue-based HIV surveillance surveys is useful to assess CT and GC prevalence, although specimen adequacy may be a limitation.
